Cleft chin surgery

What is a cleft chin surgery?

A cleft chin is a genetic trait usually referred to as a Y-shaped dimple in the middle of the chin. In many cultures a cleft chin is thought to be a sign of beauty. Today we usually associate this trait with masculinity, and thus, men usually want to increase cleft chin, while women, on the other hand, wish to get rid of this masculine feature. There are a few options which  allow to modify a cleft chin and either improve or reduce the prominence of the cleft.

To create a chin cleft the doctors use either liposuction or traditional surgical methods. They remove a tiny bit of your soft tissue and create a dimple in the chin. In some cases, the surgeon may need to remove a portion of a lower jaw.

What causes a cleft chin?

The prominence of your chin cleft is determined by your lower jaw bone, mentalis muscle (which covers the bone), and fat tissue which sits between the skin and mentalis muscle. A lower jaw bone (which is called a mandible) usually has  two prominent bumps. However, they do not contribute much to the formation of a cleft chin and are rather just the attachment points for the mentalis muscle. Mentalis is a paired muscle which lies on the mandible forming a Y shape. The depth and the thickness of the central groove depends on the distance between the two parts of the mentalis muscle.

Different approaches of a cleft chin surgery

There are a few different approaches to improving a cleft chin. They vary in price, invasiveness, and permanence.

Firstly, you can choose dermal fillers which usually last from 6 to 12 months. Soft tissue fillers, such as Hyaluronic Acid, are not only safe and reversible but also takes just 10 to 20 minutes to inject. Depending on the depth of the cleft, your plastic surgeon will determine how deep the filler should be injected. Other injections are also commonly used in removing a cleft chin. These injections ‘freeze’ the chin muscle, and thus, minimize the appearance of the cleft. As it only works for a few months it is usually combined with other fillers.

Patients can also opt for a fat grafting procedure. During this procedure a patient’s own fat is used to fill in the cleft or dimple in the chin. The results are known to be more permanent, however, it may increase the size of the chin and some patients may not feel so positive about that. The overall process is relatively simple. After a small amount of fat is extracted from the abdomen or other body areas, it is then refined and injected into the chin.

If soft tissue fillers are not effective in removing the cleft, or if a patient wishes to achieve a significant change, a doctor may recommend undergoing genioplasty. Surgery can help you to either create a cleft chin or remove it. In order to achieve the desired look the doctor can alter either the soft tissue, location of the chin muscles, or the position of the lower jaw bone. Permanent results are the biggest advantage of genioplasty, however, undergoing surgery requires anaesthesia and a longer recovery period. It may take up to a month to see the final results.

Preparing for cleft chin surgery

Being thoroughly prepared for your surgery will allow you to enjoy a stress-free recovery. Note that taking care of your mental wellbeing is just as important as being prepared physically. Thus, ask someone to assist you for at least a few days after your surgery. Arrange some entertainment options, easy-to-prepare meals, and plenty of pillows you could rest on.

As for the health aspects, you should review your medications and dietary supplements at least two weeks before the scheduled surgery. Patients are recommended to cease using aspirin, ibuprofen, CoQ10, glucosamine chondroitin, flaxseed, vitamin E, fish oil, green tea, chia seeds, primrose oil, and garlic as they all increase the risk of bleeding. Bear in mind that you should avoid tobacco and alcohol consumption one month before and after the surgery. These substances impair blood flow, and thus, can prolong the healing process. You should make sure you have got sterile gauze, Hydrogen Peroxide 3%, Arnica gel, vitamin C, and other supplies recommended by your doctor. You should follow  a balanced diet and consume enough vitamin C (up to 2000 mg daily dose is recommended to sustain collagen synthesis). Note that vitamin, mineral and protein malnutrition impairs all stages of immune response, wound healing, and tissue production.

One week before the surgery you should undergo any necessary medical tests. All aspects of your surgery, such as available strategies and post-operative period, will be discussed during a private consultation. A surgeon will also examine your medical history, skin quality, and other significant factors. You will have to undergo any necessary lab tests, including blood work, urine test, ECG (electrocardiogram), X-rays, or others. Before and after photos of your chin may also be taken.

On the day of the surgery, you should arrive earlier prior to your appointment and bring compression garments and any medications you might need. You should not eat or drink at least 6 hours before the surgery to prevent regurgitation when put under general anaesthesia. Try to get a good night’s sleep and keep calm before surgery. You should not wear any makeup, wigs or hairpieces. Patients are recommended not to have any nail polish or gel manicure on their nails. Were comfortable clothing and opt for a button-up shirts and jumpers, since changing clothes over your head may cause some difficulties.

Recovery after cleft chin surgery

Most patients experience little downtime and usually return to regular activities in about a week. After your surgery you should expect to have some swelling and bruising which will last for about 2 weeks. Expect to take 5 to 7 days off work and do not rush to get back to a fast-paced routine as this might lead to a complication. If non-dissolvable sutures were used, they should be removed in a week. It will take at least 3 weeks for you to resume your physical activities. Note that sun should be avoided for several weeks after the surgery as it can interfere with scar healing and damage the surrounding vulnerable skin. Once you are fully recovered you should not have any visible scars left. You will be able to appreciate close to final results approximately a month post-surgery.

In order to minimize swelling, sleep with your head elevated for at least 2 weeks. Apply cold compresses and use Arnica montana supplements to help you relieve swelling and bruising. You may have a sore throat which is completely normal since you were intubated during the surgery. Consume lots of fluids and eat soft foods for the first few days. You can have showers or baths, but be careful as you are not supposed to get your sutures wet.

Can cleft chin surgery be combined with facelift or other surgeries?

Combining a few surgeries together can not only save your expenses but can also result in a shorter travel and recovery time. Cleft chin surgery can be accompanied by other cosmetic procedures.

Cleft chin surgery is usually combined with rhinoplasty to give the face aesthetic proportions. Patients are often concerned just about the size of their nose, when in fact the size of their chin greatly contributes to making one’s nose appear bigger. In such cases even minimal adjustments of the nose will create a significantly improved facial aesthetics. 

Neck rejuvenation surgeries, such as neck lift (platysmaplasty) or neck liposuction, are also commonly combined with a cleft chin surgery since it helps to better frame a chin. 

Combining a cleft chin with a facelift will add volume to the chin and help to remove howls and grooves, making the jawline appear sharper
