What is fat transfer and how does this procedure work?

Fat transfer, also known as fat grafting or lipofilling, is a minimally invasive aesthetic procedure which enhances volume in certain parts of the body, which were full and well roundedbefore the aging process or weight changes. The main goal is to relocate the fat from undesired locations (e.g. arms, abdomen, thighs or other) and inject it into areas that lack definition (e.g. breasts, face, buttocks). This procedure enhances contour-lacking body parts while reducing the fat in an undesired area.

This procedure is comprised of a few steps.

Where can fat be transferred to?

Places that are usually treated with fat transfer are buttocks, breasts, legs and face. Other treatment areas may also include hips, front and back sides of the hands. Even the male chest can be enhanced in a way that it appears well trained. Fat grafting can also be used to alter intimate body parts, increasing the fullness of the female labia majora. Fat transfer is also proven to be an effective treatment for scars left after injuries, or other surgeries. Superficial scars can be filled in and effectively diminished, leaving the skin smooth and firm. Fat injections can even restore youthful hand appearance by simply injecting it into the upper side of the hands.

What results can be expected?

The biggest advantage of fat transfer is that it is a natural way to enhance one’s  appearance. Body’s own fat cells are used in transplantation which means that the overall transplant rejection rates are significantly lower than  those of synthetic fillers. Although results may vary in individual cases, under optimal circumstances fat grafting can provide a great enhancement and can be permanent. If injected correctly, fat cells will last a lifetime. It is completely natural that a portion of the injected fat is reabsorbed by the body during the first six months, however, as much as 50 percent of the fat cells will survive and continue to thrive in the new tissue. For long-lasting results, patients are recommended to repeat the treatment a few times. An experienced plastic surgeon will know the best techniques to transfer the fat cells so that their vitality is kept at their highest.

Preparing for surgery

At first, you should schedule your initial appointment. During this consultation you will discuss with a surgeon what are the exact goals. You will be evaluated as a fat grafting candidate. Being completely honest during the consultation is key since the way you present your wishes may have a great impact on the treatment plan. 

You can also bring a few pictures from the past which may serve as a good guide for the surgeon. Your medical history and health status will also be reviewed with the emphasis on any recent cosmetic procedures, current medications, allergies, pre-existing health conditions, or other risk factors. 

In order to determine your fitness for a fat transfer, any necessary lab tests, including blood work, urine test, ECG (electrocardiogram), and others, may be performed. Note that smoking and alcohol consumption is forbidden 6 weeks before and after the surgery since nicotine is known for its restrictive effects on blood circulation.

Various drugs and herbs, such as aspirin, ibuprofen, CoQ10, glucosamine chondroitin, flaxseed, vitamin E, fish oil, green tea, chia seeds, primrose oil, and garlic, can all increase the risk of bleeding. 

Your hospitalization time depends on the length and extent of the fat transfer. In order to prevent regurgitation during surgery, you should not eat or drink at least 6 hours before receiving general anaesthesia.

Arrive early to the appointment and make sure to bring any necessary medications you might need additionally.

Wear comfortable clothing and choose button-up shirts since they are easier to put on and take off.

Recovering after fat transfer

When you wake up from anaesthesia you will receive information about the care of drains (if they have been placed) and how to take care of the surgical sites. Your doctor will explain the normal postoperative symptoms that you may experience. How much time it takes for you to recover from fat grafting depends on the scope of the surgery.

Surgical sites will swell and you may experience moderate pain which can be controlled by taking anti-inflammatory drugs. Allowing yourself some downtime is crucial, therefore, take time off work and social events for at least the first 2 weeks. On the second week, bruising should subside, however, many patients will still experience some swelling later on . By the third week, you will be able to wear make-up to touch up any discoloration or bruising which can still be present at this time. Once bruising and swelling have fully receded, you will be able to appreciate the look you were going for.

Bear in mind that fat transfer success highly depends on one’s weight. Meaning that if you start losing weight after receiving a fat transplant, a fat cell will not be able to survive and will  lead to an unsuccessful fat transplant. It is crucial to maintain the body weight you were at the time of surgery so that the best results can be achieved.
