What is rhinoplasty?
A nose job, also known as a rhinoplasty, is a procedure which improves shape and function of the nose, by modifying bone or/and cartilage. Rhinoplasty can enhance aesthetic appearance as well as breathing function.
A rhinoplasty should be considered by patients who are unsatisfied with their nose size, shape or width, or those who have visible humps, wide or asymmetrical nostrils. Rhinoplasty can also help patients with obstructed airways by adjusting a deviated septum, which is a common cause of many breathing problems.
During rhinoplasty a patient is usually put under general anesthesia. Therefore, the patient will have to stay in a hospital overnight.
Who is a proper candidate?
Different rhinoplasty techniques
A closed rhinoplasty, also known as a scarless rhinoplasty, is a surgery more suitable for minor adjustments of the nose. With this technique, surgeons can sculpt and shape the nose without having to cut the nose open. Deviated septum surgeries are usually performed with this technique, too. A closed rhinoplasty is advantageous and preferred by most surgeons and patients because the incision and further alterations of the inner structures of the nose are performed through the nostrils, so there are no external scars left.
An open rhinoplasty is usually performed in more complex cases. The incision is made in the columella between the nostrils, so a surgeon can open soft tissue envelope and visualize internal structures. While commonly performed in revision and reconstructive surgeries, it is usually unnecessary for primary cosmetic rhinoplasty.
A filler rhinoplasty is a non-surgical procedure, which can correct minor imperfections of the nose. When injected, a filler can correct a nosebump ?by filling in the area around it. However, the results are not permanent and the procedure requires no less than 3 separate procedures to make a lasting change.
An augmentation rhinoplasty is a procedure which can add size to the depressed or deficient parts of a patient’s nose. For augmentation, a surgeon uses a patient’s own cartilage which is harvested from a nasal septum, the ears or the patient’s ribs. Depending on each specific case, the surgeon might choose to perform nose augmentation with closed or open rhinoplasty.
During consultation, a patient and a surgeon should define specific expectations and goals. The surgeon will suggest the most suitable technique and inform the patient about the plan of the surgery in more detail.
Function vs. aesthetics
A functional rhinoplasty is usually performed in order to restore breathing through the nose. Reconstruction of obstructed airways might be required because of congenital reasons (meaning that it is hereditary) or after traumas, such as a nose fracture. Because nostrils tend to narrow with age, older people might have difficulty breathing, and thus, might need a reconstruction of their airways.
On the other hand an aesthetic rhinoplasty focuses mainly on improving the visual appearance of the nose. This includes reducing bulbous nose tip, hump, narrowing wide nostrils, reducing the size of the nose, or vice versa, augmenting the nose.

Revision rhinoplasty
A revision rhinoplasty is needed when a patient is unsatisfied with the results of a former nose surgery and desires additional improvements. Note that before undergoing a revision rhinoplasty, the nose should be completely healed, there should be no signs of inflammation caused by previous surgery and scar tissue formation is complete. Depending on each case, a surgeon will reset the nasal bones and reposition cartilage. Airway obstruction can also be cleared by adjusting the position of the internal nose structures. It’s best if the revision surgery is performed by the surgeon who performed the first surgery.
Preparing for rhinoplasty
The patient might have very specific expectations for the outcome of the surgery, therefore, a thorough discussion with a surgeon is necessary, so each case could be evaluated individually.
Skin condition and wound healing greatly depends on nutrition. Therefore, enough proteins and vitamins such as zinc, vitamin A and C, which are important for the synthesis of collagen, should be consumed. Note that vitamin, mineral and protein malnutrition impairs all stages of immune response, wound healing and tissue production.
It is essential to consult with the doctor a couple of weeks before the surgery as you might be instructed to stop taking medications such as aspirin or other blood-thinning drugs.
Smoking and alcohol consumption should be avoided one month before and after the surgery so that it does not affect the healing process by restricting blood circulation.
In order to make your surgery experience as comfortable as possible, ask someone close to stay with you during the first few days after your surgery. Prepare ice-cold compresses, as they are excellent at relieving pain and inflammation. Stock up on lip balm as your lips will probably get really dry from having to breath through the mouth. Note that you will not be able to wear glasses. If prescription glasses are needed, you should have contact lenses prepared beforehand.
Medical examination and a review of medical history is an essential step before surgery. To determine the approach of a rhinoplasty, a surgeon will examine a patient’s nose as well as its proportion to the face and discuss available strategies. The patient will have to undergo any necessary lab tests, including blood work and/or a urine test, ECG (electrocardiogram), X-rays, or others. The surgeon will explain in detail the plan of the surgery and recovery. Before and after photos of your nose will also be taken.
In order to prevent regurgitation during surgery, an empty stomach is required for anyone undergoing general anaesthesia. Therefore, patients should not eat or drink 6 hours before surgery. Patients should try to get a good night’s sleep and keep calm before surgery.
On the day of the surgery patients should not wear any make up, wigs or hairpieces. Comfortable and loose clothing is best in this case. A button-up top would be much more comfortable to wear after the surgery than trying to put on clothes over your head.
Recovering after rhinoplasty
Depending on the technique chosen for the procedure, a patient will have either dissolvable or non-dissolvable sutures inside the nose or columella. The dissolvable sutures are supposed to dissolve after approximately a week and do not require to be removed. If the stitches are non-dissolvable, they should be removed by the surgeon 7-8 days after the surgery. The patient should also expect to have external and internal nasal dressings which will be placed inside and outside of the nose. A small gauze is placed right under the nose so that it absorbs any blood discharge, which is normal and common after rhinoplasty surgeries. Bear in mind that in the first few days after surgery, an absorbent gauze is required to be changed as often as 10 to 20 times. Nasal dressings should be removed in 5-7 days.
Patient’s upper part of the face is likely to be swollen, especially the eyes and the cheeks. Approximately 2 weeks off work are recommended to rest and heal properly because swelling and discomfort usually subsides in that time. After two weeks post-surgery swelling is usually not as noticeable and should not interfere with any daily tasks. Final results can be appreciated after residual swelling is gone, which usually takes about 6-12 months.
It is not recommended to wear eyeglasses since it puts pressure and disrupts the blood flow. In order to reduce swelling overnight, it is advisable to elevate one’s head with multiple pillows.
The patient should avoid bending forward and lifting heavy objects or children and high-intensity activities should be limited for at least 8 weeks after surgery.

Combinations with other procedures
Combining several procedures in one surgery is a good way to lower travel expenses and reduce combined recovery time. Bear in mind that when a few procedures are performed together a risk of complications and infection also gets higher. For a successful procedure and recovery it is crucial to choose a surgeon who is well experienced in all procedures that will be performed. Common combinations are:
A straightening of the deviated nasal septum can be performed in combination with improving the visual appearance of the nose;
Although it is possible to have rhinoplasty combined with breast surgery, there is a bigger chance of infection. Each case should be discussed individually in order to evaluate any specific risks;
A nose surgery and a facial improvement is a common combination of the procedures, that allows to achieve a more youthful and proportioned appearance in one surgery.
