What is a circumferential tummy tuck?
A circumferential tummy tuck, also known as 360 degree tummy tuck or circumferential abdominoplasty (lower bodylift, belt lipectomy), is a more extensive procedure than a standard tummy tuck. It may sound new, but the technique is actually an old one, however, few surgeons practise it due to its difficulty.
Abdominal dermatolipectomy/abdominoplasty has a long history. There are scattered reports of circumferential dermatolipectomy from the 20th century, but they are scarce and that type of surgery was performed by very few surgeons. Around the year 2000, bariatric surgery experienced a boom which has led to the onslaught of a new group of patients, collectively referred to as “massive weight loss patients”. Because this group of patients experienced body deformities, which occured all around the body, circumferential dermatolipectomy has quickly become a mainstream operation in plastic surgery.
Who is the perfect candidate for a 360° tummy tuck?
A flat stomach requires hard work. In some cases even a balanced diet and exercise cannot result in a tight and beautiful tummy. This may occur after pregnancy or the aforementioned massive weight loss, when the amount of excess skin is so extensive, the body cannot cope with it by itself. Then people decide to turn to surgery professionals. If discomfort occurs mainly due to sagging skin on the lower part of the abdomen and the patient wishes to have a flatter tummy and a slimmer waist, then a simple tummy tuck is normally enough. A circumferential tummy tuck is a more complicated procedure and is mostly offered to patients who have experienced massive weight loss and are unhappy with sagging skin both in the abdominal and back area, so they require changes all over their body. Thus a 360 tummy tuck can help make dramatic changes that are not possible to achieve with conservative treatment methods.
There are three groups of patients that can potentially benefit from bodylift/belt lipectomy. They are discussed in detail below:
Massive weight loss (MWL) patients make up the majority of patients who undergo bodylift/belt lipectomy. The lower torso of MWL patients can be thought of as a balloon. As patients gain weight and then lose it, the balloon is initially stretched by the weight gain then deflated as weight loss ensues. Like a balloon that has been inflated for a long time, the intrinsic elasticity of the skin is irreversibly altered during this process, leading to redundant lax skin, which is almost always circumferential in nature.
Women with a BMI range of 26–28 who have never lost any significant weight despite reasonable exercise and nutritional habits are the second group that can potentially benefit from bodylift/belt lipectomy. These patients normally have unevenly distributed fat tissue on the lower torso that is circumferential in nature, which leads to generalised lack of definition of the lower torso.
A third group that may benefit from bodylift/belt lipectomy are normal weight patients who ordinarily would be considered candidates for an abdominoplasty, but desire more dramatic improvements in lower torso area. These patients often desire a remarkable improvement in their thigh, buttock and lower back area. In many similar patients, liposuction can improve all of these areas when combined with an abdominoplasty, however, if patients desire significant lifting and contouring, then a circumferential procedure is required. A subgroup of normal weight patients that can benefit from a bodylift/belt lipectomy is made up of older patients whose skin will not contract with liposuction due to skin laxity and will require the pull created by the circumferential excision.
Preparing for the surgery
To begin with, the patient has to schedule a consultation with the surgeon. The surgeon will examine patient’s general health, prepare an individual treatment plan, and evaluate any possible risks.
Bear in mind that the patient might be asked to perform any necessary lab tests, including blood work, urine test, ECG (electrocardiogram), X-rays, or others.
The surgeon will also discuss patient’s previous medical history, any ongoing diseases, and drugs (aspirin, supplements or any hormonal medication) that are being taken by the patient. It is also important to prepare mentally as well as physically. It is not uncommon to feel uncomfortable when thinking about upcoming surgery. To make the procedure and hospital stay a pleasant experience, patients might ask someone close to them to assist them during time of recovery.
Patients are advised to stop smoking and avoid consuming excessive amounts of alcohol approximately one month before surgery. It would also be beneficial to ensure a well-balanced diet and to exercise regularly in order to speed up wound healing.

Even though circumferential abdominoplasty is more complex, the recovery period is similar to that of abdominoplasty. Patients normally spend two nights at the clinic. Drainage tubes are placed after surgery in order to collect fluids that the body produces when the wounds are healing. They are normally removed the second day after surgery. In some cases, if the body is producing an excessive amount of fluid, the drainage tubes may be kept a bit longer. A patient will be prescribed painkillers, if required.
